New Patient Forms

BEFORE COMPLETING ANY PAPERWORK YOU MUST SCHEDULE AN APPOINTMENT!, PLEASE CALL US AT (913) 353-4802 TO BOOK YOUR APPOINTMENT, AFTER THIS APPOINTMENT IS CONFIRMED PLEASE CHOOSE FROM THE FOLLOWING OPTIONS TO COMPLETE YOUR PAPERWORK:

Option 1 : Submit your paperwork electronically

• After you have made your appointment you will receive a text message with a link to complete your paperwork electronically. Your information will automatically update to your patient chart and you will be ready for your appointment upon arrival.

Option 2 : Fill out forms once you arrive at Kansas Dental Center

• If you choose to fill out your new patient forms the day of your visit, please arrive twenty minutes early to allow extra time to complete the check-in process. Upon arrival you will be provided with an iPad to fill out and sign your documents.

Cancellation And Reschedule Policy

Please note: Kansas Dental Center does not double book appointments, your appointment is reserved one-on-one time with the doctor and staff.  If your schedule has changed please honor our 48 hour cancellation policy and give us a call at (913) 353-4802 to reschedule.  If 48 hours notice is not given your account will be charged a $75.00 cancelation fee for appointments during regular business hours (8am-4pm) or $100.00 for late or weekend appointments (5pm-7pm or Saturdays).
